RISK AND LIABILITY WAIVER (READ AND CHECK THE BOX BELOW THAT YOU ARE IN AGREEMENT)
As legal guardian of the registered player, I herby consent to the aforementioned person participating in the Platinum Basketball Club programs, tryouts, practices, and/or games. I recognize that severe injuries can occur in any activity that is associated with basketball, and understand that it is the express interest of the Platinum Basketball Club to provide for the safety and protection of my child.  In consideration of allowing my child to play for the Platinum Basketball Club,  I hereby forever release the Platinum Basketball Club,  and/or any facility used by Platinum Basketball Club, it's officers, directors, employees, coaches and owners from all liability for any and all damages and injuries suffered by my child while under the instruction, supervision, or control of an of the above so mentioned.  As legal guardian of the aforementioned person, I hereby agree to individually provide for the possible future medical expenses which may be incurred as a result of any injury to my child sustained while training at, practicing at, competing at, and/or trying out for the Platinum Basketball Club and/or any other facility not listed which could be used by the Platinum Basketball Club.  In case of emergency, I authorize the Platinum Basketball Club staff to administer first aid to my child and/or take my child to a physician or hospital for further treatment.  This acknowledgment of risk and waiver of liability, having been read thoroughly and understood completely, is signed (by checking the box below) voluntarily as to its content and intent.
